Dental implants: when they are worth it

An implant replaces the root, not only the crown. It makes sense when the tooth cannot be saved, the bone allows it, and neighbouring teeth do not need to be cut down for a bridge.

A dental implant replaces the root of a missing tooth: a titanium or zirconia screw bonds to the bone, and a crown sits on it. It is not a tooth the day something is extracted, and it is not the only way to fill a gap. It is worth considering when the tooth cannot be saved, the bone and gum can hold it, and you would rather not prepare healthy neighbours for a bridge.

When an implant is a sound choice

The clearest case is a single gap between teeth that are otherwise healthy. A bridge means preparing the teeth on both sides, even if they have no decay. An implant leaves them alone and sends chewing force into the bone, not into the neighbours.

With several gaps in a row, implants can carry a bridge instead of a post in every space. A full arch is a different number of implants and a different choice between a removable and a screw-retained denture. That is not the same plan as a single crown and should not be sold with the same promise.

An implant does not treat grinding, a poor bite or untreated gum disease. If those stay, the new crown works in bad conditions. Inflammation and the bite come first. The implant is the last step, not the first.

When it is better to wait or choose something else

Too little bone does not always end treatment, but it changes the plan: sometimes a graft or a sinus lift is needed, and sometimes a bridge or denture gives function at lower cost and risk. Smoking and poorly controlled diabetes worsen healing. They are not an automatic ban for every person. They are a reason not to promise the same course as for a patient without those factors.

A tooth that has just been removed with an abscess is a poor day for an immediate implant if the infection is not under control. We sometimes place an implant on the day of extraction when the bone and gum are quiet and the screw is stable. Sometimes we wait months. The socket decides, not a package.

In someone who is still growing, an implant is a bad idea: the bone changes and the implant stays put. An adult who has finished growing is a different case.

The path from a gap to a crown

The consultation includes an exam, an X-ray or a scan, and a talk about what will sit on the implant: a crown, a bridge or a removable denture. Without a picture of the bone we cannot responsibly place the screw. A smile simulation on a screen does not replace that measurement.

Placement is done under local anaesthetic. Afterwards there is swelling and softer food on that side for a few days. The implant needs time to bond to the bone before it takes a full bite. A temporary crown is possible early for some people and not for others. A promise of "a tooth the same day", with no caveat about load, is imprecise: that day you may have a screw and a temporary crown, and the final crown still waits for healing.

Cleaning around an implant is a condition, not an extra. Floss, interdental brushes and review visits decide whether the gum stays healthy. An implant does not decay. The gum and bone around it can still become inflamed if plaque is ignored.

Bridge, denture, implant: there is no single winner

A bridge is faster when the supporting teeth need crowns anyway. A denture is simpler for large gaps and a smaller budget, at the cost of stability. An implant wins when you want to leave healthy teeth alone and the bone allows it. It loses when it is sold as the answer to every gap without an exam.

Questions for the consultation: how many implants, does the bone need grafting, when is the final crown, how do you clean the work, and what happens if the implant does not integrate. "They always take here" is not a medical answer.

At Dental Park we describe surgery, including removing a tooth an implant would replace, with oral surgery. Before extraction we check whether the tooth can still be treated: that is what root canal treatment under a microscope is about. The shade of implant crowns is chosen separately, including after whitening, because an implant crown will not change colour with the natural teeth.
